Terms and Conditions for Affordable Connectivity Now
By accessing or using Affordable Connectivity Now ("the Website") to apply for the Lifeline Program, you agree to the following terms and conditions ("Terms"). Please read these Terms carefully as they constitute a legally binding agreement between you and Affordable Connectivity Now.
1. Consent to Use Personal Information
By submitting an application through the Website, you consent to the collection, use, storage, and sharing of your personal information, including but not limited to your full name, address, date of birth, social security number (or equivalent identification number), eligibility criteria, and usage data, to assess your eligibility for the Lifeline program. You authorize us to transmit your information to authorized third parties, including government agencies, service providers, and program administrators, to verify eligibility and enroll you in the Lifeline program.
You understand that the information provided may be shared with the Universal Service Administrative Company (USAC), the National Verifier, and other regulatory bodies involved in administering the Lifeline program, in accordance with federal and state laws.
2. Eligibility Verification
The Lifeline program is a federal benefit that provides discounts on telecommunications services for eligible low-income individuals. Eligibility is determined through the National Verifier, a third-party entity operated by USAC. By submitting your application, you authorize us to check your eligibility through this verification system.
If you qualify based on the information provided, you may receive a mobile phone, tablet, or SIM card, depending on your eligibility and the specific terms of the Lifeline program. Qualification does not guarantee the receipt of a specific device, and the type of device issued will depend on availability and the Lifeline provider's policies.
If you do not qualify for the Lifeline program, you acknowledge that we are unable to provide any services, and there is no recourse or compensation available.
3. No Guarantee of Service
Affordable Connectivity Now provides services on a best-effort basis to assist you in determining your eligibility for the Lifeline program. However, we do not guarantee that you will be approved for Lifeline benefits, nor do we guarantee the availability of specific devices (such as phones, tablets, or SIM cards) if you are approved.
You understand and agree that if you do not qualify for the program, Affordable Connectivity Now is not liable for any damages or losses resulting from your ineligibility or the denial of Lifeline benefits.
4. One Lifeline Benefit Per Household
You acknowledge and agree that only one Lifeline benefit is available per household. A household is defined as any individual or group of individuals living together at the same address who share income and expenses. Applying for multiple Lifeline benefits for the same household is a violation of federal law and may result in disqualification from the Lifeline program, legal penalties, or both.
5. Program Transferability
The Lifeline benefit you receive is non-transferable. You may not transfer your benefits to another person or household. If you wish to change your Lifeline provider, you may do so in accordance with federal regulations, but your benefits will not be shared across multiple providers simultaneously.
6. Termination of Lifeline Benefits
You agree to notify Affordable Connectivity Now and/or your Lifeline provider within 30 days if you no longer meet the eligibility criteria for the Lifeline program. This may include changes in income or participation in government assistance programs that make you ineligible for Lifeline benefits. Failure to notify the provider may result in de-enrollment from the program and potential penalties.
7. Service Activation and Usage Requirements
Once your application is approved and your Lifeline benefits are activated, you must use the service at least once every 30 days. This usage may include making or receiving calls, sending text messages, using data, or responding to direct outreach from your Lifeline provider. Failure to use the service within 30 days may result in suspension or termination of your Lifeline benefits.
8. Electronic Consent
By submitting your application electronically, you consent to the use of electronic records and signatures in connection with your Lifeline application and the processing of your benefits. You acknowledge that you have the right to withdraw this consent at any time before the activation of your service. However, withdrawing consent may delay or prevent the processing of your Lifeline application.

9. Limitation of Liability
Affordable Connectivity Now, its affiliates, partners, and service providers shall not be liable for any direct, indirect, incidental, or consequential damages arising from or in connection with your use of the Website, the application process, or your participation in the Lifeline program. This includes but is not limited to any loss of eligibility, denial of benefits, device malfunctions, or service interruptions.
In no event shall Affordable Connectivity Now’s liability exceed the amount of any service fees paid by you to us (if applicable) for the specific Lifeline service in question.
10. Indemnification
You agree to indemnify and hold harmless Affordable Connectivity Now, its officers, directors, employees, and affiliates from and against any and all claims, damages, liabilities, costs, or expenses (including attorney fees) arising out of or related to your breach of these Terms, your violation of any law or regulation, or your participation in the Lifeline program.
11. Governing Law
These Terms shall be governed by and construed in accordance with the laws of the United States and the state in which you reside, without regard to its conflict of law principles.
12. Modification of Terms
Affordable Connectivity Now reserves the right to modify these Terms at any time. Any changes will be posted on the Website, and continued use of the Website and application process after such changes constitutes acceptance of the modified Terms.
